PROJECT SUMMARY/ABSTRACT
Over 2 million US adults with alcohol use disorder (AUD) are hospitalized each year, yet less than half receive
post-hospitalization AUD treatment. Current hospital-based approaches, including brief intervention and referral
to treatment, are ineffective at improving post-hospitalization linkage to care for patients with AUD. Innovative
approaches to address this gap are urgently needed. Care navigation combined with motivational interviewing
have successfully improved post-hospitalization treatment engagement among adults with opioid use disorder.
However, differences between alcohol and opioid use disorder treatment warrant adaptation of this approach
specifically for AUD. Consideration of local contexts and cost is needed to maximize likelihood of future adoption
to routine practice and long-term economic sustainability. My long-term goal is to become a national leader in
improving outcomes for patients with AUD through adaptation and evaluation of cost-effective and scalable
interventions. The objective of this proposal is to adapt and pilot the Navigation for Prioritized Alcohol Treatment
after Hospitalization (NavPATH) intervention. NavPATH will consist of motivational interviewing and care
coordination delivered in the hospital and after discharge to improve linkage to outpatient AUD treatment among
hospitalized patients. Mentored by an excellent team of experts with complementary expertise in AUD,
implementation science, clinical trials, and economic analysis, I will pursue the following aims: 1. To adapt a care
navigation and motivational interviewing intervention to AUD and local context through process mapping. 2. To
pilot test the adapted NavPATH intervention to improve linkage to post-discharge AUD care among hospitalized
patients. 3. To perform a cost analysis of NavPATH using time-driven activity-based costing. This work will
provide novel insights by qualitatively describing failure modes (i.e., barriers) and points of opportunity in the
post-hospitalization linkage to AUD treatment process and estimating the potential costs of the NavPATH
intervention. Completion of this proposal will provide me with the training and experience needed to submit an
R01 proposal to evaluate clinical and cost-effectiveness of NavPATH in a fully powered clinical trial and launch
my career as a leader in improving outcomes for patients with AUD.
