PROJECT SUMMARY/ABSTRACT
Persons living with HIV (PWH) have experienced greater life expectancy and outcomes with the advent of
highly-effective antiretroviral therapy. However, PWH continue to experience an increasing burden of non-AIDS
related comorbidities and elevated risk of hospitalization. Importantly, frequent hospitalization is common
among PWH and often reflects unmet clinical needs and disruptions in care continuity. Outpatient HIV care is
associated with improved HIV outcomes and reduced comorbidity burden, but engagement varies widely.
While some PWH maintain consistent engagement, others experience fragmented care, increasing their
vulnerability to acute health events. Despite this variability, it remains unclear which patterns of care are most
strongly associated with hospital readmission risk, and how these associations differ across patient subgroups.
Moreover, the relative contribution of outpatient care (compared to other factors such as comorbidities, immune
function, and index hospitalization characteristics) to readmission risk is not well understood. To reduce the
burden of hospitalization, it is essential to understand the relationship between outpatient and inpatient care to
inform targeted interventions. Thus, this project will evaluate the dynamic, longitudinal relationship between
HIV care and hospitalization with advanced longitudinal and machine learning methods. To accomplish these
goals, this project will analyze existing data from the Centers for AIDS Research Network of Integrated Clinical
Systems (CNICS), a clinical cohort of >50,000 PWH across 10 US sites. With mentorship from leaders in
CNICS and experts in HIV and advanced methods, this F31 fellowship will support the candidate's training in
advanced longitudinal analysis, machine learning, and implementation of research in clinical care settings. The
specific aims are to: 1) Compare standard measures of HIV care engagement and their associations with time
to first hospitalization; 2) Compare measures of post-discharge HIV care engagement and their associations
with readmission risk; and 3) Identify key predictors of hospital readmission. Lindsay is supported by a strong
team of HIV researchers and methodologic experts who will ensure the success of this project and her training
goals.
