Diabetes affects >10% of adults QUART-DT (QUantifying uptake And Real-world impacT of Diabetes Treatments) will quantify real-world coverage and fairness of advanced diabetes treatments, including sodium-glucose cotransporter-2 inhibitors, glucagon-like peptide-1 receptor agonists, and sensor-augmented insulin pump therapy, measure the gap between randomized controlled trials and effectiveness in routine care (“voltage drop”), and model population health impact across Japan, Ireland, and the United Kingdom using harmonised health data.
We will introduce two decision-ready indicators: an equitable scale-up index that summarises fair access, and a voltage-drop measure that compares matched effects from trials and real-world studies. The work will use the concentration index with the Erreygers correction for equity analysis, target-trial emulation with new-user, active-comparator designs, and state-transition modelling to estimate disability-adjusted life years and prevented events. Outputs will include open protocols, reusable analysis code, and concise dashboards and policy briefs for guideline developers and payers.
Training: The fellowship provides practical training in advanced pharmacoepidemiology (target-trial emulation and robustness diagnostics), equity methods, population-impact modelling, and natural-experiment designs, alongside transferable skills in research integrity, data governance and protection, communication, and project and budget management, structured through a career development plan.
Alignment and career objective: The project is international, interdisciplinary, and intersectoral, integrates open science and public and patient involvement, and targets societal benefit by showing who benefits, by how much, and under what delivery conditions. The fellowship will prepare the researcher to lead equity-focused diabetes health services and policy research and to contribute to data-driven guideline and policy development in Japan and internationally.
