[R01] Pesticides, Inflammation, and Lung Health: A Life Course Approach to Exposure

Pesticide exposure during pregnancy and early life is widespread and many affect lung development, contributing to long- term respiratory health disparities. Previous studies have linked prenatal pesticide exposure to respiratory disease. However, few have examined how cumulative exposures from pregnancy through adolescence affect long- term lung health, particularly among those residing in urban areas. This is a unique area, where pesticide use and respiratory disease burden intersect.
We propose to address this critical gap by leveraging a prospective urban birth cohort, PROGRESS (Programming Research in Obesity, Growth, Environment, and Social Stressors) based in Mexico City. This cohort provides detailed exposure data, bio specimens, and more than a decade of repeated lung function measurements from birth through adolescence.
We will examine associations between prenatal, childhood, and adolescent exposures to pesticide chemical classes, which include organophosphates, pyrethroids, neonicotinoids, herbicides, and longitudinal respiratory health trajectories from childhood through adolescence. We will use pesticide biomarker data from pregnancy (2nd trimester) and repeated urine samples (ages 4-5, 10-11, 13-15 years) to evaluate associations with lung function (pre- and post-bronchodilator function) and asthma symptoms over time. Pesticide exposures may trigger immune dysregulation and chronic inflammation during critical windows of development. As a result, we will characterize the relationship between prenatal and early- childhood pesticide exposure and inflammation biomarkers, cytokines, in preadolescence (ages 10-11, 13-15 years). We will analyze cytokine levels (e.g., IL-6, IL-10, TNF-á) in biospecimens collected during preadolescence to explore inflammation as a key biological response. Additionally, we will assess whether inflammation biomarkers mediate the relationship between early- life pesticide exposure and adolescent respiratory outcomes using causal mediation models.
Conducting this study in Mexico City is cost-effective and strategically important. Findings will provide insights into critical exposure windows and can inform translational strategies and interventions. Given the high relevance to US urban populations, findings can inform translational strategies and interventions.
This study will generate evidence to support public health policies that reduce pesticide exposure and promote respiratory health across the life course. By identifying modifiable risk factors early in life, the study contributes to disease prevention and improved child health worldwide.
